Range of Motion Assessment for Baseball and Softball Players
Today I want to talk about what we look for during an arm care assessment for baseball and softball players.
One of the first questions I ask is whether the athlete has any pain.
Is there pain in the shoulder or elbow while throwing?
Does it hurt during weight training?
Does it hurt at rest?
These answers help direct the rest of the evaluation. If an athlete is already experiencing pain, we may need to spend more time examining a specific area before moving on to performance testing.
If there is no pain, we typically begin by assessing range of motion.
Shoulder Range of Motion
At the shoulder, I typically evaluate:
External rotation
Internal rotation
Shoulder flexion
Horizontal adduction
External Rotation
External rotation is one of the most important measurements for overhead athletes.
There is not one perfect number that every athlete should achieve, but most healthy baseball players will demonstrate somewhere between 115 and 130 degrees of external rotation in their throwing shoulder.
Internal Rotation
For internal rotation, I generally like to see approximately 60 degrees.
While this number is important, it is actually not the measurement I pay the closest attention to.
Total Arc of Motion
The most important measurement is the total arc of rotation, which is calculated by adding external rotation and internal rotation together.
Ideally, we want this total to be close to 180 degrees.
For example:
120 degrees of external rotation plus 60 degrees of internal rotation equals a total arc of 180 degrees.
On the other hand:
120 degrees of external rotation plus only 40 degrees of internal rotation equals a total arc of 160 degrees.
Research has shown that maintaining a normal total arc of motion is associated with a lower risk of shoulder injuries in baseball players.
I should mention that most of this research has been performed on baseball athletes. While softball throwing mechanics are different, I generally use many of these same concepts when evaluating softball players as well.
Shoulder Flexion
Next, I assess shoulder flexion.
This simply refers to how high an athlete can raise their arm straight overhead.
Ideally, I would like this measurement to approach 180 degrees.
Many baseball players demonstrate slight limitations here because of the repetitive adaptations that occur with throwing.
Softball players often have closer to full shoulder flexion, although that is not always the case.
Horizontal Adduction
Finally, I assess horizontal adduction.
A simple clinical goal is to see whether the athlete can comfortably bring their elbow across the body so that it visibly crosses the line of the nose.
This gives me valuable information about the mobility of the posterior shoulder, particularly the rotator cuff and surrounding soft tissues.
Elbow Range of Motion
Evaluating the elbow is a little more straightforward, but it is just as important.
I assess:
Flexion
Extension
Supination
Pronation
Flexion
Flexion is simply how far the elbow bends.
Fortunately, this is rarely a limitation in baseball or softball players.
Extension
Extension refers to how straight the elbow can become.
Loss of extension is much more common, especially in pitchers.
There are many potential causes, including swelling after throwing, biceps tightness, joint stiffness, or even bone spurs in more advanced cases.
Maintaining elbow extension is important for both performance and injury prevention.
Supination
Supination is the ability to rotate the forearm so the palm faces upward, or toward the ceiling.
Ideally, I like to see at least 90 degrees of supination.
Pronation
Pronation is the opposite movement, rotating the forearm so the palm faces downward.
For most athletes, I like to see at least 85 degrees of pronation.
Why Does All of This Matter?
Range of motion testing gives us an objective way to identify restrictions before they become injuries.
Many times, athletes begin losing motion weeks or even months before they ever experience pain.
By identifying these changes early, we can address them with targeted mobility work, strengthening exercises, and adjustments to throwing volume before they develop into a more significant problem.
